    Please take the time to fill out this form as completely and as detailed as possible to ensure the fastest turnaround times and the most accurate permit applications. If a permit is rejected due to incomplete or incorrect information provided on this form then additional fees will be added to the final invoice to cover the added labor for repeat applications. All client and project information provided is always confidential. Thank you for your business and we look forward to working with you.

    Please ensure this field is entered accurately with the sales order number for this project.  It should be the letter S followed by 5 numbers, example "S00026".
    Only select "Yes" If you have your own access control contractor's license and will be applying for this permit without 3B Security Consulting's assistance or supervision. Otherwise, select "No".
    A clean set of plans that include the entire footprint of the building. Small sections of plans that do not show all paths of egress will not be approved during the plan review.
    Marked-up floor plans that are clear and legible. They must include the location of all access control doors, door hardware, readers, request to exit sensors, peripheral devices, power supplies, door controllers, fire drops, elevator control connections, and the direction of egress for each opening.
    A detailed door hardware schedule that matches the information on the marked-up plans. Let us know if you would like us to send you a template in spreadsheet form.
    Spec Sheets for all equipment must show either UL-294, UL-294B, or ETL listing. No exceptions will be made by the plan review engineers. (Doors that are fire rated will also require fire rated locking hardware that are UL-10C listed to match or exceed the rating of the wall they are installed on)
    5. A copy of the quote for your client. Used for cross-referencing the markups and door hardware schedule to ensure the accuracy of the permit plan application.

    This amount is used to determine the fee for the permit that is paid upon approval of the application for permit. It must include parts, labor, and profit associated with the sale and installation of the access control system.
    Projects that significantly affect the structure and design of a historic or landmark building require additional fees and a different permit process.
    Whether or not a building is classified as a high rise affects code requirements.
    The # of stories a building has affects code requirements.
    Whether or not a building is fully sprinkled affects the fire rating required locking hardware installed on fire rated doors.
    This only effects elevator lobby doors that will have access control. The building engineer or property manager will likely have this information.
    All 120VAC connections to power supplies require a licensed electrician to do the work. If a power supply comes from the factory with a plug installed an electrician is not needed to plug it into the wall :) Please just enter N/A if one is not needed for this project.
    A current and valid contractor license # is required for the permit application.
      All maglocks, stairwell doors, elevator lobbies, and delayed egress hardware require a fire alarm interface. The fire alarm contractor will be needed onsite for testing, pre-inspections, and final inspections with the city. Please just enter N/A if one is not needed for this project.

    All connections made to an industrial elevator control unit must be made by a licensed elevator technician. Please just enter N/A if one is not needed for this project.
